Platelet count

Platelet count is a routinely performed laboratory investigation that measures the number of circulating platelets (thrombocytes) within the blood. Platelets are small, anucleate cell fragments derived from bone marrow megakaryocytes and play a fundamental role in primary haemostasis, vascular repair, and thrombus formation. Measurement of platelet count forms part of the full blood count (FBC) or complete blood count (CBC) and is one of the most frequently requested laboratory tests in clinical practice.

Assessment of platelet count provides valuable information regarding haemostatic function, bone marrow activity, inflammatory responses, and a wide range of haematological, infectious, hepatic, and systemic disorders.

Physiological Role of Platelets

Following vascular injury, circulating platelets adhere to exposed subendothelial collagen through interactions mediated by von Willebrand factor (vWF). Activated platelets subsequently aggregate and release procoagulant mediators, promoting activation of the coagulation cascade and formation of a stable haemostatic plug.

Adequate platelet number and function are therefore essential for maintaining normal haemostasis. Abnormal platelet counts may increase the risk of haemorrhage or thrombosis and frequently provide an important diagnostic clue to underlying disease.

Thrombocytopenia

Thrombocytopenia refers to a reduction in circulating platelet count below the laboratory reference range. The risk of spontaneous bleeding generally increases as platelet counts decline, although bleeding risk also depends on platelet function, the underlying cause, and coexisting abnormalities of coagulation.

Common causes of thrombocytopenia include:

  • Immune thrombocytopenia (ITP)
  • Viral infections
  • Sepsis
  • Disseminated intravascular coagulation (DIC)
  • Bone marrow failure or infiltration
  • Chemotherapy and radiotherapy
  • Drug-induced thrombocytopenia
  • Chronic liver disease with hypersplenism
  • Thrombotic microangiopathies

Clinical manifestations may include petechiae, purpura, ecchymoses, mucosal bleeding, epistaxis, gingival bleeding, menorrhagia, or prolonged bleeding following trauma or invasive procedures. Severe thrombocytopenia may be associated with life-threatening haemorrhage.

Thrombocytosis

Thrombocytosis describes an elevated platelet count above the upper reference limit and may be classified as reactive (secondary) or primary (clonal).

Reactive thrombocytosis commonly occurs in association with:

  • Acute or chronic infection
  • Inflammatory disorders
  • Iron deficiency
  • Recent surgery or trauma
  • Malignancy
  • Recovery following significant blood loss

Primary thrombocytosis is most commonly associated with essential thrombocythaemia and other myeloproliferative neoplasms.

Although many patients remain asymptomatic, marked thrombocytosis may increase the risk of arterial or venous thromboembolism. In some cases, particularly with extremely elevated platelet counts, acquired von Willebrand syndrome may develop, resulting in an increased risk of bleeding.

Platelet Count in Clinical Assessment

Platelet count should always be interpreted alongside the wider clinical picture and complementary laboratory investigations. Assessment commonly includes:

  • Haemoglobin concentration
  • White blood cell count and differential
  • Blood film examination
  • Coagulation profile
  • Liver function tests
  • Renal function tests
  • Inflammatory markers
  • Bone marrow examination where indicated

Serial platelet measurements are often more informative than isolated results, particularly in critically ill patients, individuals receiving cytotoxic therapy, or those with evolving haematological disease.

Platelet Count in Clinical Scoring Systems

Platelet count forms part of several validated clinical scoring systems and prognostic models used throughout modern medicine.

It contributes to the Sequential Organ Failure Assessment (SOFA) Score, where platelet count is used to evaluate coagulation dysfunction in critically ill patients. Platelet count is also incorporated into several liver disease severity scores, haematological prognostic models, and risk stratification tools used in hepatology, oncology, and intensive care medicine.

The inclusion of platelet count within these validated models reflects its importance as an objective marker of haemostatic function and disease severity.

Clinical Interpretation

Reference ranges vary slightly between laboratories; however, platelet counts in healthy adults generally fall between 150 × 10⁹/L and 450 × 10⁹/L. Interpretation should always consider the patient’s clinical presentation, medication history, comorbidities, and associated laboratory findings. Apparently abnormal results should also prompt consideration of laboratory artefacts such as EDTA-induced platelet clumping (pseudothrombocytopenia), which may produce falsely low platelet counts.

Clinical Significance

Platelet count remains an essential component of routine clinical assessment and plays a central role in the investigation of bleeding disorders, thrombotic disease, bone marrow pathology, infection, liver disease, and critical illness. When interpreted alongside clinical findings and complementary laboratory investigations, platelet count provides valuable diagnostic and prognostic information that supports evidence-based clinical decision-making across a broad range of medical specialties.
