Atrial fibrillation in adults: diagnosis, stroke prevention and management

Clinical scope and immediate priorities

Atrial fibrillation is a common sustained arrhythmia in adults and is clinically important because it is associated with thromboembolism, heart failure, impaired quality of life and excess mortality. Management is not a single decision about heart rate. A useful consultation separates five questions: is the rhythm truly atrial fibrillation, is the patient unstable, what is the risk of stroke, what is the risk of bleeding, and is the principal symptom strategy rate control, rhythm control or a combination over time.

Clinical urgency comes before long-term scoring. Patients with haemodynamic compromise, ongoing myocardial ischaemia, acute pulmonary oedema, syncope with instability or another immediately dangerous presentation require acute assessment according to local emergency pathways. In a stable patient, the task is to document the rhythm, identify precipitating or associated disease, assess symptoms and establish a plan for stroke prevention and arrhythmia management. Paroxysmal atrial fibrillation still carries clinically important thromboembolic risk; an intermittent pattern should not be assumed to be benign.

Confirming the diagnosis and characterising the arrhythmia

An irregular pulse should prompt a 12-lead ECG when atrial fibrillation is suspected. A single normal ECG does not exclude paroxysmal disease. If symptoms are intermittent, ambulatory ECG monitoring should be matched to symptom frequency so that the likelihood of capturing an episode is reasonable. The record should describe whether atrial fibrillation is newly detected, paroxysmal, persistent or long-standing, while recognising that these categories can change as the clinical course evolves.

Initial assessment should look for clinically relevant contributors and consequences. History commonly addresses palpitations, breathlessness, exercise tolerance, chest symptoms, presyncope or syncope, alcohol exposure, intercurrent illness, thyroid disease, sleep-disordered breathing and medication use. Examination should include cardiovascular status and evidence of congestion. Investigations are individualised but commonly include renal function, electrolytes, full blood count and thyroid testing, with echocardiography when structural information will influence management. Atrial flutter is assessed through a related but not identical pathway, and other irregular rhythms should be distinguished before treatment decisions are made.

Stroke-risk assessment and anticoagulation

Stroke prevention is one of the highest-value components of atrial fibrillation care. NICE recommends formal thromboembolic risk assessment using CHA2DS2-VASc in adults with atrial fibrillation, including paroxysmal, persistent or permanent atrial fibrillation and atrial flutter. The score supports a structured conversation rather than replacing judgement. Age, prior stroke or transient ischaemic attack, heart failure, hypertension, diabetes, vascular disease and sex category contribute to the estimate, but the final decision also considers comorbidity, treatment burden and patient preference.

When anticoagulation is indicated, direct-acting oral anticoagulants are generally considered before vitamin K antagonists unless there is a reason they are unsuitable or contraindicated. Choice between agents depends on factors such as renal function, interacting medicines, adherence, bleeding profile and licensed indications. Mechanical heart valves and some forms of valvular disease require a different anticoagulation approach. Aspirin monotherapy is not an alternative stroke-prevention strategy for atrial fibrillation. Anticoagulation decisions should be revisited when clinical circumstances change rather than treated as a permanent one-time calculation.

Bleeding risk, modifiable factors and safe prescribing

Bleeding risk should be assessed at the same time as thromboembolic risk, but a higher bleeding score is not in itself a reason to withhold indicated anticoagulation. NICE recommends the ORBIT bleeding risk score for people being considered for, or already receiving, anticoagulation. The practical value of the exercise is to identify modifiable contributors and improve follow-up. Examples include uncontrolled blood pressure, interacting antiplatelet or non-steroidal anti-inflammatory medicines, harmful alcohol use, anaemia and potentially avoidable combinations of antithrombotic therapy.

Before prescribing, check renal function, body weight where relevant, concurrent medication and the licensed dosing criteria for the selected anticoagulant. Doses should not be reduced simply because bleeding is feared if the person does not meet the medicine-specific reduction criteria. Conversely, renal deterioration, frailty, acute illness or newly introduced interacting medicines may create a need for reassessment. Patients should understand what to do about missed doses, bleeding symptoms, planned procedures and over-the-counter medicines, and they should know which service to contact when advice is needed.

Rate control and rhythm control

Rate control is a suitable first-line strategy for many adults, particularly when symptoms are modest and there is no strong reason to restore sinus rhythm. Drug choice is influenced by left ventricular function, exercise status, comorbidity and tolerability. Beta blockers and rate-limiting calcium-channel blockers are commonly used in appropriate patients; digoxin has a more specific role and is less effective for controlling exercise-related tachycardia. Combination therapy may be required, but bradycardia and conduction disease must be considered.

Rhythm control should be considered when symptoms persist despite rate control, when atrial fibrillation is likely to be contributing to heart failure, when there is a potentially reversible trigger, or when the clinical context favours restoration of sinus rhythm. Cardioversion, antiarrhythmic medicines and catheter ablation are specialist decisions shaped by arrhythmia duration, structural heart disease, symptoms, recurrence risk and patient preference. Anticoagulation requirements around cardioversion must be handled carefully because restoration of sinus rhythm does not immediately remove thromboembolic risk.

Follow-up and reassessment

Atrial fibrillation management benefits from planned review. Reassess symptoms, ventricular rate, treatment tolerance, anticoagulant adherence, renal function where appropriate, new vascular or bleeding risk factors and any change in the person’s goals. A person who initially chose rate control may later develop a stronger indication for rhythm management, while a patient who has undergone apparently successful rhythm treatment may still require anticoagulation because stroke risk is determined by the underlying risk profile rather than the visible rhythm at one appointment.

Communication between primary care, anticoagulation services, cardiology and pharmacy is particularly important around hospital discharge, procedures and medication changes. The record should make the reason for anticoagulation, the selected agent and dose, and the review plan explicit. If uncertainty remains about anticoagulation, complex valve disease, recurrent symptoms or candidacy for ablation, specialist cardiology input is appropriate.

Practice points and limitations

The most avoidable errors are failure to confirm the rhythm, failure to assess stroke risk, using bleeding risk as a simple exclusion test, under-dosing anticoagulants without licensed criteria, and assuming successful cardioversion or ablation automatically removes the need for stroke prevention. Local formularies and peri-procedural anticoagulation protocols should be followed because operational details vary.

Documentation, shared decisions and transitions of care

Good atrial fibrillation care depends on making the reasoning visible in the record. Document the ECG evidence or monitoring result that established the diagnosis, symptom burden, stroke-risk assessment, bleeding-risk assessment, relevant renal function and the rationale for the chosen antithrombotic and rate or rhythm strategy. Where anticoagulation is not started despite a clinically meaningful thromboembolic risk, record the reason and the plan for reassessment. Where treatment is declined, record the information discussed and the person’s preference rather than leaving an unexplained gap in care.

Shared decision making should cover the outcomes that matter to the individual. Some patients are primarily concerned about stroke, some about bleeding, and others about symptoms, exercise tolerance, medication burden or the prospect of procedures. Explain that stroke risk and symptom control are related but separate management problems. A person can feel well and still benefit from stroke prevention, while a person with substantial symptoms may need rhythm-focused treatment even when ventricular rate appears acceptable in clinic.

Transitions of care are a vulnerable point. Hospital discharge information should clearly state the anticoagulant, dose, indication, start date, renal considerations, any planned review and whether antiplatelet treatment is also intended. When cardioversion or ablation is planned, responsibility for peri-procedural anticoagulation should be explicit. Community pharmacists, anticoagulation services and primary care teams can identify adherence problems, interacting medicines and inappropriate dose changes when information is shared promptly.

Review should also consider prevention beyond the arrhythmia itself. Optimising blood pressure, heart-failure treatment, weight, alcohol intake, diabetes management and sleep-apnoea assessment where indicated can reduce the wider cardiovascular burden and may influence recurrence. Symptoms should be interpreted against baseline functional status, because older or less active patients may describe fatigue or reduced exercise tolerance rather than palpitations. Where uncertainty persists, a focused symptom-rhythm correlation can help determine whether atrial fibrillation is truly responsible for the patient’s limitation before a procedural rhythm strategy is pursued.
